- List the names of authorized visitors along with their National Identity Card numbers.
- Provide signatures for both the candidate and the father or guardian, along with dates.
- In the certificate section, fill in the details required by the head of the educational institution currently being attended.
- Review the rules and regulations provided, and ensure understanding, as you will need to acknowledge them.
- Complete the undertaking section by filling out the required details and signing it.
- Before submitting, ensure that you have attached all required documents such as photographs and identification copies.
