9. If applicable, address any environmental non-compliance actions in section eight.
10. Finally, review the certification statement in section nine, sign and date the application to confirm that the information provided is accurate.
11. Gather all additional required documentation, including identification, course completion certificates, and your 2' x 2' photo. Include appropriate payment for the fees.
12. Submit the completed application form, along with all supporting documents and payment as indicated in the instructions.
