- In Part 2: Skills and Experience, indicate the LCI-directed trainings you have facilitated and the corresponding years. Describe your experience in group facilitation, technology, teaching, and public speaking, providing details of when these skills were used and the audience involved.
- In Part 3: Professional History Relevant to Facilitation Experience, state your relevant field or profession and outline your primary responsibilities.
- In Part 4: Additional Information, list any other skills or experiences that may qualify you as a Lions Certified Instructor. Additionally, explain why obtaining this certification is important to you.
- In Part 5: Acknowledgements, review and check each statement to confirm your understanding of the program’s commitments. Include your signature, printed name, and date.
- Once you have completed all sections of the form, you can save your changes, download a copy, print it, or share it as needed. Finally, email the completed form to LCIP@lionsclubs.org before the deadline of January 31, 2020.
