13. Complete Annex ‘A-1’ regarding particulars of testing instruments, providing purchase receipts for verifications.
14. Fill out Annex ‘A-2’ detailing the staff employed, and attach attested copies of their qualifications and national identity cards.
15. Indicate the fee of Rs. 5000/- paid into NBP, including branch details and treasury receipt number along with the date.
16. Review your declaration statement to affirm that all particulars are correct and that you understand the terms and conditions associated with the license. Make sure to attach a duly signed copy of these terms.
