- Ensure that the total percentage of benefits allocated to all beneficiaries equals 100%. This step is crucial for the validity of your form.
- Complete the initialing requirement. The member and two witnesses must initial on the designated lines provided.
- In the signatures section, indicate the place, sign, and date. If the member cannot read or write, a thumbprint is acceptable instead of a signature.
- Finally, ensure all pages of the form are complete and valid before submitting. You can then save changes, download, print, or share the form as needed.
