- Read the Acceptable Use Policy, complete the fourth page of this document, then scan and save it as a PDF with the filename format: 'AUP - First Last.pdf.'
- Compose an email to ltc.operations@tn.gov, attaching both the Tpaes Access Request Form and the Acceptable Use Policy, and set the subject line to 'TPAES User.'
- After sending your email, be aware that processing access requests may take up to 10 days.
