- Fill out Part Seven, detailing all relatives entitled to notice. If no relatives are entitled, check the box provided.
- Proceed to Part Eight to indicate whether all attorneys are applying for registration. If not, provide the necessary details.
- In Part Nine, confirm whether you have enclosed the registration fee and express any wishes to apply for fee remission.
- Certify the information in Part Ten by having all attorneys sign and date the form.
- If applicable, provide correspondence address in Part Eleven, specifically if it differs from Attorney One’s address.
- Finally, use Part Twelve for any additional information to support your application, attaching extra pages if necessary.
