14. If applicable, state how long you have resided in the Riccarton/Wigram ward.
15. In section four, provide information about yourself or your team, including details of involvement in your sport or activity, achievements, future goals, and how your participation will benefit you and the community.
16. In section five, declare that all information provided is true and sign the form. Your parent or caregiver must also sign if you are under 18.
