- Specify any other funding sources you have applied to for this event, along with the amount requested and the date of the decision.
- Describe any fundraising activities you have undertaken or are planning for this event.
- Record how much funding you or your team has raised so far.
- Indicate whether you have applied to the Community Board for assistance before.
- If applicable, state how long you have resided in the Riccarton/Wigram ward.
- In section four, provide information about yourself or your team, including details of involvement in your sport or activity, achievements, future goals, and how your participation will benefit you and the community.
- In section five, declare that all information provided is true and sign the form. Your parent or caregiver must also sign if you are under 18.
- Return your completed application to the Shirley Service Centre or post it to the designated address.
