Unlock a protected document On the Review tab, in the Protect group, click Restrict Editing. In the Restrict Formatting and Editing task pane, click Stop Protection. If you are prompted to provide a password, type the password.

Check the screen to ensure the correct headings are shown. Do these match the heading styles you have applied. Also check if the TOC is set to display enough levels of headings. Increase the levels by clicking on the drop down list.

If you've manually applied formatting to any of your titles, they might show up in the TOC. But a title in a document with a "Title" heading (or "Subtitle" heading) generally won't appear in the TOC (or outline). Instead, use only Headings 1-6 for your titles.

The TOC field may have been locked. Try selecting the Table of Contents and then press CTRL+SHIFT+F11 (unlock field) and see if you can then update the Table of Contents. Another possibility is that the Table of Contents has been unlinked from the data in the document.

Go to References > Table of Contents > Custom Table of Contents. Select Modify. If Modify is grayed out, change Formats to From template. In the Styles list, click the level that you want to change and then click Modify.

Go to References>Table of Contents>Insert Table of Contents. If you've already got a table of contents, customizations will be applied. If not, this will insert a new customized table.

Put your cursor where you want to add the table of contents. Go to References > Table of Contents. and choose an automatic style. If you make changes to your document that affect the table of contents, update the table of contents by right-clicking the table of contents and choosing Update Field.
