After the third week of the degree-expected term, students can access the MyUCLA Diploma Request feature or use a Diploma Mail Request form to provide diploma release instructions and select a delivery method.

The Degree Audit is available through MyUCLA and on request from the Student Services Office, 3250 Public Affairs Building. Students should review their Degree Audit with an undergraduate adviser to ensure that all requirements will be satisfied.

You may purchase a replacement diploma by downloading and completing the Replacement Diploma Request and mailing this form with the required $10.00 fee per requested diploma to USF. Please allow 15 business days for delivery of your new diploma to a domestic address and 30 business days for international destinations.

Current students may request these services on CalCentral under the My Dashboard tab > Student Resources > Submit a Form> Verification Request. Alumni and former students with access may request these services on CalCentral: Forms > Submit a Form > Enrollment/Degree Verification.

If you lost or misplaced your original Berkeley College diploma or certificate, and would like to request a replacement copy, complete the Credential Request Form. There is a $20.00 fee per diploma plus the shipping cost ($15.00 for domestic shipping; $90.00 for international shipping).

For graduating students, verification of a degree can be issued after the degree has been posted to the student's record, approximately six weeks after the end of the term. If earlier proof of degree is required, contact a degree auditor in 1113 Murphy Hall and complete an Early Certificate of Completion Request.

Most UCLA graduates can use the MyUCLA Diploma Request feature or a Replacement Diploma Request form to order a replacement diploma if the original has been lost or destroyed, or to request re-issue of a diploma after a graduate's legal name change.
