Birth Certificates are $25.00 each plus the credit card service fee. Standard shipping via US Postal Service is free. If you are requesting your order to be shipped outside the United States please call (513) 732-7499 to make arrangements. Birth Certificates prior to 1908 cannot be ordered online.

The fee for certified copies of birth or death certificates is $25 per copy. Methods of Payment include cash, check, credit card, or money order. Credit Card purchases require an additional processing fee. Due to changes in State law, we can no longer provide uncertified copies of birth or death certificates.

County Access to Birth Certificates: Birth certificates can be obtained from the Illinois County of Birth Records Registrar more quickly that from the Department of Public Health Vital Records Office.

To Place an Order Complete, print, and sign the appropriate form below. Include $24 for the first copy of the certificate and $15 for each additional certificate, plus the VitalChek processing fee noted on the application. VitalChek accepts payment by personal check, credit card, or money orders.

Certificates may be picked up same-day from the office between 8am-4pm. Orders not picked up by 4pm will automatically be mailed the next business day. Phone orders may also be placed by calling VitalChek at 1-866-550-1853. An additional $11 transaction fee is added to process your credit card order.

If you need to obtain a certified copy of a birth or death certificate from the state of Ohio, you may contact the Ohio Department of Health(opens in a new window) for statewide birth certificates, or contact the Department of Health or Vital Statistics in the county the birth or death took place.

The simplest way to get a certified copy of a birth or death record is to order online using a credit card. Our online order portal now accepts Visa, Mastercard, Discover, and American Express cards. Orders are processed in our office within five business days.

Canadian birth and death certificates must be obtained from the vital statistics office of the province in which the event occurred. They cannot be obtained through the Canadian Embassy. Requests should be sent directly to the appropriate provincial vital statistics office.
