Ensure that the Home tab is the active tab. Click the New Items icon on the New group of the Ribbon, point to More Items and then click Choose Form. Click the Look In: drop-down list and choose User Templates in File System. Click the template you want to use. Click Open.

To add a custom field into the body of your email, click + Text Field, add in the field name, and press OK to insert. To add a drop down menu, click the +Dropdown button, fill in the field and menu options, and press OK.
