Generally you can choose how you want your name displayed on a certificate. However, it is customary to have at least your first name and family name spelled in full. Any other names can be substituted with just the first letter.

Most commonly, people write out their first name and surname, and use the initial letter of the middle name, as: Donald J. Trump. Some people, however, go by their middle name rather than first name, in which case the middle name is spelled out and the first name is reduced to an initial, as: D. John Trump.

In the U.S., the middle name is often abbreviated to the middle initial (e.g. Mary Lee Bianchi becomes Mary L. Bianchi). This is usually standard for signatures or omitted entirely in everyday use (e.g. just Mary Bianchi). An individual may have more than one middle name, or none.

List the highest education degree first, for example, Michael Anderson, PhD, MSN. In most cases, one degree is enough, but if your second degree is in another relevant field, you may choose to list it. For example, a nurse executive might choose Nancy Gordon, MBA, MSN, RN.

When a professional has earned more than one set of post-nominal letters, it is appropriate to display each set of letters after his or her name. This is done in descending order, with the most prestigious letters being first (closest to the name), followed by a comma, then the next set of letters and so on.

Your surname is your family name. It's also called your ?last name.? When filling out applications, type your surname as it appears on your passport, travel or identity document. Do not use initials.

First name will always be your birth name. Surname is second name, mostly your fathers name. Sometimes family name is also added as second surname.

The initials of name are meant to be written in the front. You can ask the concern department of school to correct it.

If you have a degree, start by listing the highest degree you've earned immediately after your name, such as a master's degree, bachelor's degree or associate degree. If you have multiple degrees, you may choose to list only the highest degree you have earned since this often eclipses previous degrees.
