- Press the ‘Get Form’ button to access the form and open it in your digital editor.
- Begin by entering the county where the property is located in the designated field. If applicable, you'll need to complete separate forms for properties spanning multiple counties.
- In the municipality field, input the name of the municipality corresponding to the property. If the property is located in various municipalities, a Supplemental Form will be necessary.
- For the Grantee/Purchaser section, fill in the last name, first name, and middle initial for up to two purchasers, followed by the Social Security Number or federal ID where required. Make sure to also provide the mailing address, municipality, state, and ZIP code after the purchase.
- In the Grantor/Seller section, similarly provide the required details for the seller(s), including their names, identification numbers, and mailing addresses.
- For the Property section, enter the map, block, lot, and sub-lot numbers as applicable. If the property requires special documentation, indicate that no maps exist or detail multiple parcels as necessary.
- Specify the type of property by entering the corresponding code in the designated space based on the classification provided in the instructions.
- Indicate the physical location of the property, including street and number, as well as the acreage if applicable. Keep in mind that if the transaction is a gift, lines relating to property type and acreage can be omitted.
- In the Transfer Tax section, input the purchase price. If this transfer is a gift, enter '0'. If applicable, also state the fair market value for tax assessment purposes.
- Check the exemption claim box if relevant, and provide an explanation for this exemption in the specified area.
- Enter the date of transfer in the MM-DD-YYYY format to reflect the date ownership is officially transferred.
- Complete the Classified section if the property is enrolled in special use programs such as tree growth or working waterfront.
- If there were any special circumstances affecting the sale price, check the relevant box and provide a detailed explanation.
- Fill in the income tax withheld section, confirming that the buyer is not mandated to withhold Maine income tax under outlined scenarios.
