Form CT-W4, Employee's Withholding Certificate, provides your employer with the necessary information to withhold the correct amount of Connecticut income tax from your wages to ensure that you will not be underwithheld or overwithheld.

Withholding is the money an employer withholds from each employee's wages to help prepay the state income tax of the employee. An employer must withhold Connecticut tax if the employee is a resident of Connecticut, performing services in the state.

Form CT-W4, Employee's Withholding Certificate, provides your employer with the necessary information to withhold the correct amount of Connecticut income tax from your wages to ensure that you will not be underwithheld or overwithheld.

Choose the adjustment process if you want the amount shown on line 21 credited to your Form 941, Form 941-SS, or Form 944 for the period in which you file Form 941-X. Check the box on line 1. OR Choose the claim process if you want the amount shown on line 21 refunded to you or abated. Check the box on line 2.

As a general rule, there is a ten year statute of limitations on IRS collections.
