  9. In Line 8, enter $5 if you are paying your quarterly tax due in installments and have filed timely, noting that this fee is only payable with the first installment.
  10. Add up amounts for Lines 5, 6, 7, and 8 to complete Line 9a, and if less than $1, submit the report without payment.
  11. If choosing installment payments, enter the due amount for this quarter only on Line 9b.
  12. List each employee’s social security number in Line 10, ensuring all entries are accurate and complete.
  13. In Line 11, enter the last name, first name, and middle initial for each employee.
  14. Complete the certification section, ensuring the report is signed by the appropriate party such as the owner or authorized agent.
  15. Enter the gross wages paid to each employee this quarter on Line 12a, and then calculate the total gross wages on Line 13a.
  16. Lastly, review all entries for accuracy and completeness before saving, downloading, printing, or sharing the form.
