- Section III: Compact termination. Mark the appropriate boxes to indicate the reason for termination, such as adoption finalized, reaching majority age, or if legal custody is returned to parents or relatives, attaching any necessary court orders.
- If the placement has completed treatment, mark that item. Also indicate if the sending state's jurisdiction has been terminated with the concurrence of the receiving state, or if termination was unilateral.
- Provide the exact date of termination under Section III and check any applicable boxes based on the child's situation.
- Section IV: Signatures. Have the designated person sign under 'Person/Agency Supplying Information', noting their agency and the date. The Compact Administrator or a designated alternate should sign and date in the second block.
- Finally, distribute the form correctly. Ensure that you retain necessary copies while sending the completed original and the required copies to the appropriate administrators.
