Follow the steps to successfully submit your claim.

1. Click ‘Get Form’ button to access the claim form and open it in your preferred editing tool.
2. Provide your membership number in the designated section at the top of the form.
3. In section A, enter your personal details. Include your title, surname, first name(s), address, postcode, telephone number, mobile number, email address, and date of birth.
4. If the claimant differs from you, complete the details in the 'Your claimant details' section. Include the same information fields as in step 3.
5. Indicate whether the claim is a result of a third-party accident or injury by marking 'Yes' or 'No'.
6. In section B, fill out your benefit payment method. Provide the account holder's name, name of the bank, bank account number, and sort code.
7. Sign and date the authorisation for direct credit under section B.
8. For section C, tick the box next to the benefit you are claiming for and complete the required subsections as indicated.
9. In section D, attach a copy of the original itemised receipt supporting your claim and keep the original for your records.
10. Complete any additional details required in sections E to G based on your claim type, such as providing birth certificates for maternity claims or death certificates for funeral grants.
11. In section H, if applicable, authorise the hospital to share your admission details and have it signed by the authorising officer.
12. Review and carefully read section I, then sign and date the member declaration to confirm all information is true and complete.
13. Finally, save your completed form, download it for your records, and print or share as needed.
