- Make sure to review the Claim Submission Requirements provided at the bottom of the form. Attach all necessary original documents such as official receipts, itemized bills, and certificates as specified for Out Patient and In-Patient Treatment.
- Read and complete the Authorization Statement confirming the truthfulness of your submitted documents and authorizing Metlife to process your reimbursement.
- Finally, sign and date the form in the Employee’s Signature section. Ensure all information is accurate before submission.
