- If an LTC advance was not applied for, indicate whether you informed the Accounts Officer (A.O.) Claims about your LTC plans.
- Confirm whether your to and fro tickets are booked. If yes, remember to include a copy.
- Specify the leave applied for, ensuring to attach a copy of the application or relevant documentation.
- In section 7(a), state the leave encashment claimed or applied for. Specify the number of earned leave days.
- In section 8(b), provide information on the leave at credit by stating the number of days for Earned Leave and Half Pay Leave, including the necessary HRMS statement as an enclosure.
- Indicate whether this is your first time applying for leave encashment by selecting YES or NO.
- Fill in the number of occasions you have availed of leave encashment earlier.
- If applicable, indicate whether a joint declaration has been submitted if your spouse is also a government servant.
- Provide your pay and scale of pay on the date of application in the required format.
- Finally, ensure you sign the form to validate your application. Check all enclosed documents and confirm they are attached.
