Enter account name, bank account number, country, bank account currency, and account type by selecting from options like checking or savings. Ensure no punctuation, dots, dashes, or spaces are included in your bank account number.
- If applicable, fill in intermediary or correspondent bank information, including the bank name, address, bank account number, and SWIFT code.
- At the end of the form, you will need to authorize the agency to direct payments to your specified bank account. Include your signature and title.
