Follow the steps to successfully complete the Qnet Registration form.

1. Click ‘Get Form’ button to obtain the Qnet Registration form and open it in your preferred editor.
2. Fill out the required fields clearly and completely, including the request date, first name, middle initial, last name, business email address, job title, employer name, Medicare provider number (if applicable), Joint Commission ID number (if applicable), setting, employer address, work phone number, and fax number.
3. Select and answer a security question that is easily remembered but hard for others to guess, and fill in the corresponding answer.
4. As the applicant, sign and date the form in the presence of a Notary Public. Be sure to have the Notary's signature and seal on the form as required.
5. Obtain the signature of the highest-level executive at your location on the QualityNet Exchange Administrator Authorization form, which is attached to the registration form.
6. If applying as a Vendor or Health Care System, mail the original completed forms to the QNet Help Desk. If not, mail to your QIO or ESRD Network, making sure to retain a copy for your records.
7. Wait for the QualityNet Help Desk to process your registration. You will receive a notification email once processing is complete, along with your Log-In ID.
8. If you have not received your initial password, click the 'Forgot Your Password?' link on the Log-In screen of the Qnet Exchange website to receive a temporary password via email.
9. Follow the instructions found in the Resources/Getting Started/System Setup section of the QualityNet Exchange website to complete your system setup.
10. Run the Test Your System feature to ensure that your computer meets the requirements for using the QualityNet Exchange.
