Fill Act 24 Clearance

Act 24 requires all current school employees to complete a Pennsylvania Department of Education (PDE) form to report prior arrests or convictions. This form is required with submission of clearances every 5 years. Act 24 Arrest Conviction Report, Completed Physical Exam Form, 2025 W-4 I-9, Employment Eligibility Verification, Direct Deposit Authorization. All volunteers who are subject to background check clearances are required to re-certify their clearances every FIVE years. Submit a copy of the clearance to Human Resources. 4. Please note: This clearance must be valid Thru the Semester of your Field Experience. 7. These clearances must be done once every five years.
