Follow the steps to complete your Samplogic form

1. Click the ‘Get Form’ button to access the Samplogic form and open it in your browser.
2. Select your option: To submit a collection request, double-click the 'Sample Booking' icon. Alternatively, if you are looking to track the status of your samples, enter the Airway Bill Number or the VPS Order Number in the 'Track & Trace' box and press submit.

6. Indicate the number of samples to be collected by entering the quantity in the specified field.
7. Enter the requestor's information, including their name and email address. Providing an email address will ensure they receive notifications about the collection and shipping documents.
8. In the pickup information section, select the country from which the sample will be collected.
9. Enter the shipper's company name. Utilize the drop-down list to select a previously saved shipper from your address book, or enter the company name and address if it is new, ensuring that these details are saved for future use.
10. Enter the city name of the pickup location.
11. After selecting the country and city, a list of postal codes will appear. You can either scroll through to find your postal code or type the first few digits to refine the list.
12. Finalize your form by accepting the VPS terms and conditions.
13. Click submit to complete your sample pickup request.
14. After successful submission, you will receive a unique VPS Order Number and Airway Bill Number, and the necessary shipping document will be sent to the email addresses you provided.
