- Indicate the type of account at the financial institution (checking or savings) and enter the provider’s account number with the financial institution.
- For each NPI bank account enrolled, tape a voided check or copy of the bank letter to the designated area on the form. Make sure to fax or mail this along with the completed form.
- If you have multiple NPI numbers, repeat steps 2 through 8 for each additional NPI, using separate addendum forms as needed.
- Once all sections are completed, review the information for accuracy before submitting. You can then save your changes, download, print, or share the filled form as required.
