- In the Was Paid section, document what the employee was originally paid. Ensure to enter the types of hours and amounts in the hundredths format (HH.HH). Use the TACS Employee All report as a reference.
- Move to the Should be Paid section and record all hours to represent what the employee should be paid for that applicable week. Like before, enter hours in the hundredths format and ensure the total adds up to 40.00 hours for the week.
- If applicable, fill out the Higher-Level or Dual Appointment section. Indicate whether higher-level pay or dual appointment is relevant, and provide the respective rate schedule code and level. Additionally, denote if the employee has a higher-level status at the end of the last scheduled day of the week.
- In the Remarks section, add any necessary supporting remarks. If applicable, include the grievance step and GATS number for any grievances.
- Complete the Issuing Office Mailing Address section by entering the authorizer's office mailing address.
- Ensure both the employee and authorizer sign and date the form. The authorizer must also print their name and include their telephone number.
- If a salary advance is needed, fill out the Emergency Salary Advance Authorization and Receipt section. Record the year and pay period, and detail the amounts being issued for both weeks, along with the 10-digit window unit ID and date of issue.
- Once the form is completed, save any changes, then proceed to download, print, or share the form as necessary.
