10. Indicate whether the well is currently in use by selecting Yes or No. If it is in use, list any other applicable permit numbers as required.
11. Collect the property owner/water purveyor's signature in the designated section.
12. If there is a NJDEP case manager assigned to your request, include their name and provide a contact phone number.
13. After you have filled out all necessary sections, attach the completed form to your well permit application. Be sure to save any changes made, then proceed to print or download your completed form for submission.
