5. Proceed to Section D for any change or transfer information. Fill in the new office name, new name change, new address change, and new email address if applicable. If transferring, provide details for the new office, board, or association.
6. Once all sections are completed, review your entries for accuracy and completeness. After confirming that the information is correct, save your changes. You may then download, print, or share the completed form as needed.
