- If requesting a non-preferred drug product, fill out Section III to provide medical necessity for opting for the non-preferred drug over preferred alternatives.
- Fill in the prescriber information, ensuring to include all required fields such as last name, first name, NPI, and contact details.
- Complete the prescribing provider’s attestation by providing a signature and date, confirming all information is true and accurate.
