A dosage form that contains one or more active and/or inactive ingredients. Medications come in many dosage forms, including tablets, capsules, liquids, creams, and patches. They can also be given in different ways, such as by mouth, by infusion into a vein, or by drops that are put into the ear or eye.

(1) The name and dosage form of the controlled substance. (2) The date filled or refilled. (3) The quantity dispensed. (4) The initials of the dispensing pharmacist for each refill.

Controlled substances are drugs or chemicals that have the potential to be addictive or habit-forming. The United States Drug Enforcement Agency (DEA) divides controlled substances into 5 categories called Schedules based upon substances' potential for abuse and addictiveness and its' usefulness in medicine.

(1) The labeling must contain a summary of the essential scientific information needed for the safe and effective use of the drug. (2) The labeling must be informative and accurate and neither promotional in tone nor false or misleading in any particular.

Prescription labels for controlled substances must contain specific information. Similar to noncontrolled prescription labels, the label must contain the pharmacy's name and address. It must also contain a prescription number, the patient's name, the prescriber's name, directions for use, and the date of filling.

◦ The general standard of practice for documenting usage of liquid controlled medications is to record the starting volume from the label, record each dose administered, subtract the dose administered from the previously recorded volume, and record the remaining amount.

The following information must be on every prescription label: Name and address of the dispensing pharmacy. Serial number of the prescription. Date of the prescription. Name of the prescriber. Name of the patient. Name and strength of the drug.
