4. In Section 3, you, or a guardian/carer, must complete the patient declaration. Confirm that the information provided is accurate, and authorize hospital staff to access necessary medical information.
5. Section 4 requests referral details. If required, enter information about the patient’s diagnosis, travel requirements, and whether special assistance or accommodation is needed.
6. In the final section, Section 5, this part is filled out by an approving officer. Ensure they complete the necessary assessments and approvals required for submission.
7. Review all the entered information for accuracy. Once satisfied, save any changes made to the form, and proceed to download, print, or share your completed application as needed.
