- In Part III, the sponsor of the account must complete their section by filling in their first name, last name, job title, department name, campus address, email address, room number, and campus phone number.
- Finally, in Part IV, the sponsor must agree to assist Newark Computing Services by reading the agreement, providing their name, signature, and date.
- Once the form is fully completed, submit it to the Newark Computing Services Help Desk via email, fax, or in person. Ensure you save changes to your document if completed online.
