- For the Strongly Recommended Immunizations, fill in the vaccination dates for any additional vaccines you have received, if necessary.
- Have your healthcare professional complete and sign the Required Immunizations section. Ensure that they provide their contact information as well.
- Sign and date the Consent and Release section, ensuring that you name an emergency contact and provide their phone number.
- If you are under 18 years old, have your parent or guardian complete the necessary sign-off in the Consent and Release section.
- Before submitting the form, ensure that all four parts are complete: Required Immunizations, Strongly Recommended Immunizations, Physician’s Signature, and Consent and Release Signatures.
- After completing the form, return it via mail or fax to the Student Health Center at the address provided on the form.
- Remember to complete the four online forms located on the portal, including the Patient Information-Medical History form and others as listed.
