1. Click ‘Get Form’ button to obtain the form and open it in your preferred document editor.
2. Begin by entering the foal's name in the designated field. If registration papers are pending from associations such as AQHA or APHA, indicate that in the appropriate section.
3. Fill in the foaling date, breed, and sex of the foal. Select 'Filly' or 'Colt' as applicable.
4. Provide the sire's name and registration number, then repeat the process for the dam.
5. Indicate whether the foal is a result of embryo transfer by choosing 'Yes' or 'No' in the corresponding box.
6. Complete the owner contact section by providing the name, address, city, state, zip code, and both home and cell phone numbers of the owner.
7. Repeat the process for the nominator contact, entering the same information as required.
8. Review the enrollment fee schedule and check the appropriate fee box based on when the form is postmarked.
9. Fill in the fax number, email, and website details where required. Note the total amount due and select your preferred payment method: check, money order, or credit card.
10. If using a credit card, fill in the account number, expiration date, CVC, name on the card, and billing address. Make sure to provide your signature to authorize the payment.
11. Once all information is completed, save your changes. You may download, print, or share the completed form as needed before submitting it to the NRBC.
