  20. Determine the tax due before discount by multiplying item 14 by 0.00331 and including the result in item 15.
  21. If applicable, calculate any discounts for report years 2008 or 2009 in item 16.
  22. Determine the total tax due by subtracting item 16 from item 15 in item 17. Remember not to include payment if this amount is less than $1,000.
  23. Print or type your name and include your area code and phone number in the designated fields.
