The Medallion Design Contest offers students an opportunity to showcase their creativity by designing a medallion for the winners of the Mohawk Valley Technology Education and Pre-engineering Showcase. This guide provides step-by-step instructions on how to complete the submission form online effectively.

Follow the steps to successfully complete your submission form.

1. Click ‘Get Form’ button to access the Medallion Design Contest Rules & Submission Form and open it in your online document editor.
2. Begin by filling out the 'Official Entry Form' section. Enter your name, grade, school, instructor’s name, and email address.
3. Next, choose one of the available templates for your design. You may select from Template #1, Template #2, Template #3, or Template #4 as outlined in the form.
4. Create your logo based on the selected template. Ensure that your design meets the contest's requirements for a custom shape if you decide to create one.
5. Convert your design into a usable format, ensuring that it is a black and white, single bit bitmap or scalable vector graphic. Acceptable file formats include cdr, dxf, ai, svg, and bmp.
