The permanently disabled children / siblings and dependent parents to whom family pension has been authorised in the PPO of the pensioner will submit this Form to the Pension Disbursing Authority.

Death Certificate of the pensioner (original plus one photocopy) the original will be returned after verification by the bank. Non remarriage certificate and non-employment certificate to be submitted. Simple declaration to be attested by the Bank official or any gazetted officer.

After a pension is sanctioned, its continuance depends on future good conduct vide Article 351, CSR [Rule 8, CCS (Pension) Rules, 1972] but it cannot be stopped or reduced for other reasons.

Form 3 is a declaration submitted by the inventor to the Indian Patent Office (IPO) in which the applicant provides all the information regarding foreign filing applications.

In case government employee died while in service,family pension will be paid at enhanced rates i.e. 50% of pay last drawn for a period of10 years. Thereafter family pension will be paid at the rate of 30% of the last pay. If family pension is authorised to parents, it will be paid at the rate of 30% of the last pay.
