3. Enroll in the Moodle site and complete the Detailed Application Information. Before doing this, prepare all necessary details to fill out this section in one sitting as you cannot save your progress. Refer to page 3 for a complete list of required information.
4. Submit your application as an 'Assignment' in Moodle. Combine all components, including your curriculum vitae, essays, and the scanned signatures page, into a single PDF file named ‘lastname_firstname.pdf’. Ensure the PDF is submitted through the Moodle Assignment tool by the specified deadline.
5. Send the original Signatures Page via campus mail to the designated recipient. Ensure that the page is sent to Dr. Melissa Bostrom at the provided address, including the relevant Campus Box number.
