Fees. Currently, the annual charge is $2.50 for each enrolled practitioner.

The Self-Query service searches the NPDB to see if the information an individual or entity provides in a Self-Query request matches any report information stored in the NPDB.

Your electronic Self-Query response is available for 45 days from the completion date. If you need to access your electronic Self-Query response after 45 days, you will need to complete a new Self-Query.

The cost per query is $2.50 for either a Continuous Query enrollment (which lasts one year) or a One-Time Query.

Once a completed Self-Query request is received by the NPDB, it may take up to one business day to process.

Adverse clinical privileges actions should not be reported to the NPDB unless they adversely affect the practitioner's clinical privileges for a period longer than 30 days. Matters not related to the professional competence or professional conduct of a practitioner should not be reported to the NPDB.

On the NPDB Account ID page, enter your Account ID, then select Continue. Your Account ID is on your notification letter. If the system displays a password or access code form, enter the code from the notification letter. If you have signed in to your report in the past, enter the password you set for your account.
