- Specify the date of first use of the tradename or trademark in MM/DD/YYYY format, ensuring the date is not more than 30 days in the future.
- Select the classification code that corresponds to your goods or services. Refer to the classification list for accurate selections.
- Sign the form in the presence of a notary public. Your printed name and title in relation to the business must also be included.
- Provide the contact person's information so they can respond to any queries related to this registration.
- Ensure that you submit the correct filing fee of $15. Payments should be made payable to WDFI. Keep in mind that registration expires after 10 years.
- If you are submitting multiple applications, each one should be accompanied by the required fee, although a combined payment is acceptable.
- After processing, a Tradename/Trademark Certificate will be issued, and confirmation will be sent via email or postal service.
