These studies support the conclusion that a normal (<37 U/mL) or low preoperative CA 19-9 serum level (<100 U/mL) correlates with early pancreatic cancer stage and independently predicts improved overall survival, whereas an elevated CA 19-9 serum levels (>100 U/mL) is associated with a poor prognosis (38-49).

In a small study by Beretta et al,15 the authors found that in seven patients whose postresection CA 19-9 never returned to normal, no patient survived longer than 7 months (median, 4.8). However, in seven patients who had a normal postoperative CA 19-9 level, median survival was 17.3 months (P < . 005).

CA-125 levels in the body are measured by units per millimeter (U/mL). A CA-125 normal range falls between 0 and 35 U/mL. Levels over 35 U/mL may indicate the presence of cancer or other conditions. Not all patients with a high CA-125 result have cancer.

The reason for CA 19-9 elevation in benign disease can be explained by several mechanisms. First of all, inflammation and proliferation of non-tumorous tissue, such as in pancreatitis, cholangitis, bronchiectasis, idiopathic pulmonary fibrosis, ovarian cyst, and endometriosis, can elevate CA 19-9.

Elevated levels (>37 U/mL) of CA 19-9 have been associated with gastrointestinal carcinomas, particularly in pancreatic cancer,17–20 and is considered to be one of the most favorable biomarkers for the management of pancreatic cancer.
