11. Proceed to fill out 'Operation Classifications' by checking all that apply. Make sure to include all relevant categories.
12. Indicate 'Cargo Classifications' by checking the appropriate types of cargo transported or shipped.
13. Complete the 'Hazardous Materials' section only if applicable.
14. Provide details on the number of Commercial Motor Vehicles (CMV) your company operates in the U.S.
15. Fill out the driver information, including the total number of drivers operating CMVs.
16. If your USDOT registration is currently revoked, indicate that by selecting 'Yes' and providing the USDOT Number.
17. Complete the 'Passenger Carrier Compliance Certification' if you are a passenger carrier.
18. Enter the names and titles of sole proprietors, partners, or corporate officers as required.
19. Finalize the form by certifying with an authorized signature, date, and printed name.
20. Review all entered information for accuracy. Save changes, and either download or print the completed form.
