Get OR OHSU Transcript Request Form

You obtain an official transcript from your previous institution in a sealed envelope that bears a seal, stamp or official signature from the institution across the back flap of the envelope. You place that envelope, unopened, into a larger envelope and mail the transcript to the University of Oregon.

Where do I send my transcripts? All applicants will need to send transcripts directly from their college to Nursing CAS: NursingCAS. P.O. Box 9201, Watertown, MA 02471. Any transcripts sent directly to the OHSU Admissions Office while applications are open, will not be accepted.

The cost to apply is $60.00 for an undergraduate program application and $80.00 for a graduate program application.

Note for Undergraduate Students: All official transcripts should be sent to the Office of Admission at 3601 Pacific Ave. Stockton, CA 95211.

We need official transcripts from all colleges you have attended or are attending. If you are currently enrolled at another college or university, wait to send your transcripts until you only have one outstanding term remaining prior to beginning your studies at the UO. For fall applicants, this would be spring term.
