  4. In line 2, enter the HSA contributions you made for 2024, including those made by the due date of your tax return, excluding employer contributions.
  5. For line 3, if under age 55 at the end of 2024, enter $4,150 (or $8,300 for family coverage) only if eligible throughout the year.
  6. In line 4, enter contributions made to your Archer MSAs for 2024 from Form 8853, if applicable, including any contribution amounts related to your spouse.
  7. Subtract line 4 from line 3 and enter the result in line 5. If zero or less, enter -0-.
  8. For line 6, if applicable, enter your additional contribution amount if you are age 55 or older at the end of 2024 and had family coverage.
  9. Add lines 6 and 7 and enter the result in line 8. Then report the total employer contributions for 2024 in line 9.
  10. Enter any qualified HSA funding distributions in line 10 and combine lines 9 and 10 in line 11.
  11. Subtract line 11 from line 8 and enter the resulting amount in line 12 as your HSA deduction.
  12. Proceed to Part II to report HSA Distributions. If both you and your partner have separate HSAs, complete a separate Part II for each.
  13. Enter total distributions in line 14a, rolling over amounts in line 14b, and subtract these results in line 14c.
  14. Enter qualified medical expenses paid using HSA distributions in line 15, and then calculate taxable HSA distributions in line 16.
  15. If any of the distributions meet exceptions to the Additional 20% Tax, check the box in line 17a.
  16. Finally, for Part III, report any income and additional tax for failure to maintain HDHP coverage, completing additional calculations and checking appropriate boxes.
  17. After ensuring accuracy, save your changes, download a copy, or print the form for submission.

IRS Form 8889 is used to report HSA contributions, distributions and your tax deductions. You will complete this form using IRS Forms 1099-SA and 5498-SA provided by HSA Bank. Instructions for Form 8889.

Form 8889 must be filed with your annual Form 1040 federal tax filing if you make contributions to or take distributions from an HSA.

If you hold an HSA account (or are the beneficiary of a deceased HSA holder), you're required to attach Form 8889 to your Form 1040 when filing your personal income tax or information return. If you fail to do so, you'll be unable to deduct your HSA contributions.

Any interest or earnings on the assets in the HSA are tax-free while held in the account. You can receive tax-free distributions from your HSA, including distributions of interest or earnings, to either pay or be reimbursed for qualified medical expenses you incur after you establish the HSA.

Employer contributions to your Health Savings Account are reported on Form W-2 Wage and Tax Statement, Box 12 with a code "W." An amount entered here will flow to Line 7, Column C of California Schedule CA. The amount is then added to the income total on Line 16 of California Form 540.
