The exemption application process for individuals and organizations is described in A.R.S. 42-11152 . In almost every case, the taxpayer must file an application with the county assessor, which provides the information required by the assessor to make an exemption determination.

Proof of medical disability and annual income shall be required and there is no qualifying minimum age restriction under this exemption. Taxes shall be exempt on a sliding scale basis up to a maximum of 50% of the assessed valuation, which percentage is determined by the owner's prior year gross income.

Obtain the claim form from the County Assessor's office where the property is located. Submit the completed form to the same office. Once the exemption has been granted, it remains effective until a change in eligibility occurs, such as selling or moving out of the home. Annual filing is not required.
