Birth Certificates The cost for the first certificate is $10.00 and $4.00 for each additional copy. Please make checks payable to the Indiana Department of Health.

Payment: You can pay by cash, money order in the exact amount, personal check, or credit or debit card in your name. The cost for each birth certificate is $23. The cost for one death certificate is $21.

You can order birth certificates, death certificates and marriage records on-line, by mail, phone or in person by visiting our Vital Records office. Birth certificates cost $17, with a $6 fee for additional copies of the same record.

The first three digits show the region code, while the next two digits usually represent the year of birth. The last six digits are usually a unique serial number depending on the year of birth. In most cases, the document number is located on the in the upper right corner.

Customers who complete an application and pay the fee will receive the birth certificate that day, unless a search is required. For death and marriage certificate applications whose fees have been paid, the certified copy will be mailed to the customer within 7-10 business days.

For information on how to get the long form, contact your county of birth or the Texas Department of State Health & Human Services (DSHS) Vital Statistics Unit at 1-888-963-7111.

Birth certificates are available from MSDH's Vital Records office when you visit in person, order by phone, or order by mail. Online ordering is also available. Changes or Corrections: Call our Vital Records office at 601-206-8200 or send e-mail to VRInfo@msdh.ms.gov.
