6. For each study, begin logging the cases chronologically from the earliest to the most recent date. Use the format dd/mm/yy for the study date.
7. In the 'Study Category' field, select from options such as LV function assessment or other categories mentioned in the guidelines.
8. Provide a 'Main Diagnosis or Conclusion' for each case. Note that only one primary diagnosis should be assigned per study.
9. In the comments section, include additional important details if necessary, but note that this field is not always mandatory.
10. After entering all data, review your entries for accuracy before submitting. Once completed, you can save changes, download, print, or share the form as required.
