8. Answer whether there have been any antibiotic residue violations during the year, and if so, provide details regarding the incident.
9. Lastly, report how many cows have been culled during the timeframe, along with the reasons for culling, particularly for udder health.
10. After completing all sections, ensure that all information is clear and complete, then save your changes. You can download, print, or share the completed form as needed.
