An Affidavit of Identity is a legal document that is used to formally confirm or verify a person's identity. It is typically sworn under oath and signed in the presence of a notary public or another authorized official.

If a person uses multiple names in different documents, but without legally changing your name, then he/she has to submit this certificate, to declare that it is only he/she has used all those names (mention all variations of your name accurately in the document). The person completing the 'one and the same' affidavit bears legal and financial responsibility should the facts in the affidavit be successfully challenged in an administrative or court proceeding. Make one register entry for an affidavit with multiple signers, but be sure to include the name of each signer in the entry. You may need to use more than one line in your register to record all of the names. Do not make a separate register entry for the oath or affirmation you administer as part of an affidavit.

In general, it is possible for two people to sign an affidavit if they are both named in it and are swearing to the same set of facts. However, cases can differ and the specific details may determine whether two people can and should sign one.
