- Still in Section C, list the courses you are currently enrolled in by noting the course code and title.
- In Section D, Course Selection, identify at least eight courses you wish to enroll in, along with the corresponding discipline code, course number, section number, credit hours, days, and times.
- Proceed to Section E, Housing, where you will select your preferred housing option, either 'On-campus' or 'Off-campus'.
- In Section F, Emergency Contact Information, provide the name, telephone number, and relationship of your emergency contact person.
- At the end of the form, certify that the information recorded is correct by signing and dating the application. Ensure the Home Institution Study Abroad Coordinator also signs and dates the form.
- Finally, submit the completed application along with all required materials to the Study Abroad or Exchange Coordinator at your home institution. This includes your official academic record, proof of English language competency, financial statements, and a photocopy of your passport's biographical information page.
- Keep in mind the application deadlines: October 1 for the spring semester, and May 1 for the fall semester or academic year. After filling out the form, you can save changes, download, print, or share the document as needed.
